AT YOUR FIRST VISIT: You will be greeted by one of our treatment facilitators who will show you around the office. After your tour, we will ask that you join us for a personalized “FREE OF CHARGE” examination.
To expedite the process and provide Dr. Statler with helpful information about you, please fill out one of the appropriate forms below prior to your visit. For your convenience you can submit this form online OR print it and bring the completed form to our office OR fax it to us at 561-395-9391.
During the exam we will take imaging photographs that will allow Dr. Statler to do a detailed diagnostic work up of your facial structure, jaw, and teeth. She will also look at a special digital xray to evaluate your growth and development. She will review these records with you and discuss a treatment plan that is specific to your goals and needs. There is plenty of time to discuss your concerns and answer any questions after your detailed consultation.
We work very hard to accommodate our patients schedules. Customer service is our focus and we are constantly trying to improve your access to care. We have extended our office hours and offer flexible time slots to make more appointments available when you need them.

FINANCIAL CONSIDERATIONS
Orthodontic treatment is an important and often necessary investment in the overall dental, medical and psychological well being for children and adults. Financial consideration should not be an obstacle to obtaining this important health service. We understand that different people have different financial needs and obligations, therefore we provide several payment options. Our goal is to make paying for your orthodontic treatment as convenient and manageable for your family as possible.
OPTION 1 – INITIAL AND MONTHLY PAYMENTS
This payment plan is a contracted agreement with our office, requiring an initial down payment, followed by monthly payments spread over the term of your anticipated treatment time. The initial payment is due on or before your first appointment, in order not to delay the beginning of treatment. We do not charge interest on this budgeted plan.
OPTION 2 – PAYMENT IN FULL
For those patients who wish to pay in full at the time treatment starts, a administrative courtesy reduction from the total fee is given. One payment in full reduces our administrative costs and we are happy to share these savings with you.
OPTION 3 – NO MONEY DOWN PAYMENT PLANS
One of these plans may be of particular interest to those who want to start orthodontic treatment without an initial down payment as required in #1. Rapid approval is usually available for these loans, which carry an interest charge and low monthly payments.
OPTION 4 – INSURANCE
Our office is happy to work with your insurance company to help you receive your maximum benefits in the most timely fashion. In most cases, we are able to accept direct payment from the insurance company
You may pay with cash, checks, MasterCard, Visa, Discover, American Express or by checking debit. Many patients choose to pay online by coming to this website, activating My Account Manager, and following the directions for payment.
